Third-Party Cybersecurity Incident

In February 2024, one of our third-party vendors, Change Healthcare, experienced a cybersecurity incident that resulted in unauthorized access to information on their network. Importantly, this incident was limited to Change Healthcare’s systems, and Premier Eye Care of Florida systems have not been compromised or impacted by this incident

As a result of the incident, Premier Eye Care of Florida was notified by Change Healthcare that data related to a limited number of Premier Eye Care of Florida patients was part of the information accessed without authorization as a result of the incident. Due to the limited information Premier Eye Care of Florida received, Premier Eye Care of Florida was unable to verify the patient information that may have been impacted as a result of the incident impacting Change Healthcare. As such, out of an abundance of caution, Premier Eye Care of Florida is providing individuals with information about the incident and the services that Change Healthcare has made available to those whose personal information may have been involved.

Notably, Change Healthcare has reported that, at this time, there is no indication that any of the information involved has been misused; however, out of an abundance of caution, Change Healthcare is offering complimentary credit monitoring services to support any individuals who believe that their information may have been affected as a result of the incident. As such, individuals can sign up for free credit monitoring by visiting: https://app.idx.us/en-US/account-creation/2E49GM5TZ

For more information regarding this incident, you can visit Change Healthcare’s website at www.changehealthcare.com/hipaa-substitute-notice.html or call their dedicated call center at 1-866-262-5342, available Monday through Friday, 8 a.m. to 8 p.m. CT.

Thank you for your continued support and patience as we worked to close this process.
